Job Description
Supports Access to Reproductive Health Services and Contraceptive Care grant focused on increasing equitable access to reproductive health resources, including assessments and immediate access to contraception, for underserved women in Marion County through a coordinated continuum of care involving formalized referral partnerships and accessible patient navigation. Serves as a bridge between the community and the health care, government and social service systems.
Assists individuals, families, groups and communities access resources, including health insurance, food, housing, quality care and health information. Provides informal counseling, support, and follow-up. Must have experience working with patients in community settings. Bilingual preferred. Grant concludes: 10/31/25.
8am-4:30pm Community Health Specialists supporting the Infant/Maternal Health program will spend 3 days working in a clinic setting and must have reliable transportation to conduct home visits, with mileage reimbursement. 1-2 days per week can be remote as long as Community Health Specialists is meeting goals and up to date on assignments. Community Health Specialists are also required to be available for community events that take place during nontraditional work hours (up to 10% of time). Most of the outreach to patients occurs in clinic or telephonically and there are times when Community Health Specialists connect in-person with patients to deliver items and provide education. Coaches may drive to 1-2 locations each day when they’re not remote. Community Health Specialists are also required to participate in team meetings/retreats, which may be in-person. Training and education will be provided as part of the orientation process.
