Cancer Centers’ Use of Survivorship Care Plans in Indiana
The ICC’s mission is to reduce Indiana’s cancer burden through the coordinated, collective action of its members and the sharing of resources, knowledge, and passion. We work across the entire continuum of care, which consists of four focus areas: primary prevention, early detection, treatment, and survivorship. The goal of the Survivorship Committee is to improve the quality of life for all those affected by cancer. Cancer survivorship aims to prevent and control adverse outcomes and provide knowledge regarding timely follow-up care, surveillance, and optimal quality of life after cancer treatment.
During the 2023 annual meeting, participants and stakeholders completed a survey rating survivorship concerns to be addressed by the ICC over the next five years. Increasing the delivery of individualized survivorship care plans was identified as the top objective. The complete Indiana Cancer Control Plan can be found at https://indianacancer.org/iccp-report/.
This survey is for the Indiana Cancer Consortium Survivorship Committee to learn if and how cancer centers in Indiana are providing patients with survivorship care plans. Our goal is to use the survey results to determine how we can increase the delivery of comprehensive, individualized survivorship care plans.
The committee has compiled a list of cancer centers and organizations in Indiana. Our committee is seeking individuals to reach out to these organizations to (1) obtain the appropriate contact to complete the survey and (2) distribute the survey. Distribution is electronic, but may also include a phone interview, if preferred by contact.
This is a short-term project and is a volunteer experience. This project is not expected to last more than two months. The prospective date of completion is based on the start date.
